Opportunity:
RN - Case Management
Job Details:
* Weekend coverage includes 5 hours for one Saturday each month with 5 hours off on one weekday in exchange
* Responsible for the ongoing development, implementation, and evaluation of processes within the Case Management Plan
* Provide advocacy, communication, collaboration, resource management, and education to interdisciplinary team members
* Maintain and improve the case management system through ongoing interaction with patients, physicians, and other health providers
* Document Case Management plans in a clear and concise manner
* Assist physicians, patients, families, staff, and payers in providing appropriate, effective, and cost-efficient patient care
* Attend and actively participate in all departmental and interdepartmental meetings related to Case Management and Social Services functions
* Collaborate with payers and review organizations to ensure proper utilization of patient care resources
* Educate physicians and staff regarding appropriate level of care and utilization issues
* Maintain current knowledge of Case Management, Social Services, discharge planning, and nursing issues through continuing education
* Bachelor's Degree in Nursing required (Master's Degree preferred)
* Minimum 3 years of clinical experience in a hospital setting required
* Minimum 5 years of case management experience in a hospital setting required
* Certification in case management preferred or willingness to obtain within 2 years
* Current unrestricted Registered Nurse license in the State of Colorado required
* Current BLS certification required
