RN - Home Health

Job Details:

* Day shift position
* EPIC experience required
* Mileage reimbursement provided in addition to base rate
* Minimum of two years of recent experience in home health or related field required
* Bachelor's Degree in Nursing (BSN) preferred
* Acute care facility or home care experience preferred
* Valid Driver's License required and must be at least 21 years of age or older
* Current automobile insurance and a reliable automobile required
* Required certifications: BLS (from an approved vendor)
* Current RN licensure in state of practice required
* Complete initial and ongoing comprehensive assessments of patient needs
* Manage a patient caseload effectively and efficiently in a home care setting
* Provide skilled, safe, and quality care independently while collaborating with other core disciplines
* Implement and update plans of care in accordance with identified patient needs
* Coordinate patient care with other disciplines and integrate input from care team members
* Accurately document all pertinent patient care information in a timely manner
* Counsel and educate patients and family members in meeting healthcare and nursing needs
* Initiate appropriate preventative and rehabilitative nursing procedures
* Serve as a resource to patients, family members, and other care team members
