Opportunity:
Weekend Field Nurse
Job Details:
* Work weekend shifts on Saturday and Sunday from 7:00 AM to 11:00 PM
* Perform participant assessments in person to determine appropriate levels of care including emergent, urgent, non-urgent, and home care status
* Respond promptly to calls from primary care providers, triage, and support staff while prioritizing needs as calls are received
* Complete field visits utilizing full body assessment skills, critical thinking skills, and electronic medical records
* Make home visits on a regular basis to provide care and treatment as ordered by primary care providers including wound care, IV hydration, lab draws, nurse assessments, and other nursing tasks
* Provide education, instruction, and support to participants, DPOA, family, friends, and facility staff
* Coordinate and collaborate with providers, staff, DME suppliers, ALF, SNF, paramedics, pharmacy, administration, home care coordinators, and hospitals
* Initiate emergency treatment and document new orders as necessary
* Complete appropriate documentation for falls, threats of suicide, medication errors, incident reports, grievances, and other quality measure policies
* Actively participate in new employee orientation, ongoing in-service programs, staff meetings, and continuous quality improvement initiatives
* Bachelor's degree in Nursing preferred; Associate's degree required
* RN Nursing Diploma or Degree from an accredited School of Nursing or University required
* Minimum of one year of experience working with frail, elderly, or long-term care population required
* Minimum of one year of home care experience preferred
* Minimum of one year of wound care experience preferred
* Current CPR certification by AHA required or must be willing to obtain
* Must have reliable transportation, a current valid Michigan driver's license, and car insurance
* Must be 18 years of age or older
