Opportunity:
Hospice RN
Job Details:
* Perform comprehensive assessments of patient needs and coordinate individualized care plans
* Provide pain and symptom management consistent with the hospice plan of care
* Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and hospice philosophy
* Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and volunteers
* Participate in on-call rotation with 1-3 shifts per week, including 3 weekends during the 13-week contract depending on staffing levels
* Must be able to respond within 45 minutes to patient needs when on call
* Maintain accurate and timely documentation in compliance with state, federal, and organizational policies
* Conduct home visits with valid driver's license and reliable transportation required
* Mileage reimbursement provided at $0.70 per mile
* Must be able to work holidays
* Minimum of 3 years of nursing experience required, preferably in hospice, home health, or palliative care
* Current RN license in the state of Utah or compact state RN license required
* Current BLS Certification required
* Hospicesoft/Careficient EMR experience preferred
