Job Details:

* Work in a split care setting: 50% Home Health and 50% Skilled Nursing Facility with flexibility required based on area need
* Monday through Friday schedule from 8:00 AM to 4:00 PM
* Guaranteed 36 hours per week
* Work week runs Sunday through Saturday
* Home Health charting software: HCHB; Skilled Nursing Facility charting software: NetHealth
* Productivity expectations: Home Health 90%, Skilled Nursing Facility 85%
* Therapy Supervisor available on site
* No on-call or call back available
* Day shift - Home Health
* BSN not required
* New graduates are welcome to apply
* Required certifications: BLS Certification
