Physical Therapist - Skilled Nursing Facility

Job Details:

* Must have skilled nursing experience required
* Bachelor's degree required, Master's or Doctorate preferred
* Active Physical Therapist license in applicable state of work required
* Basic Life Support (BLS) certification preferred
* Prior experience in long-term care or skilled nursing facility preferred
* Partner with residents and care providers to provide physical therapy services
* Perform evaluative and therapeutic procedures under physician orders
* Evaluate residents per department protocols and establish therapeutic plans of care
* Provide consultation to referring physicians and healthcare team members
* Monitor and evaluate treatment effects and progress treatment programs
* Document all evaluative and therapeutic interactions in Electronic Medical Record
* Recommend and obtain appropriate assistive devices with proper instruction
* Provide education to residents and caregivers with documented results
* Work collaboratively with Physical Therapist Assistants and other therapy professionals
* Comply with Medicare and other reimbursement regulations
* Supervise assistant-level staff when appropriate
* Maintain required licenses, certifications, and competencies
* Must be able to lift up to 50 pounds and perform combination of sitting, standing, walking, bending, stooping, twisting, and reaching
* Strong verbal, written, and presentation communication skills required
* Proficient in Microsoft Office required
