Job Details:

* Work 12-hour day shifts from 6:00 AM to 6:00 PM
* Both 8-hour and 12-hour shifts available
* Every other weekend and holiday requirement
* Post-acute care setting
* Patient ratio of 1:17
* 61-bed facility with current census of 34
* Location-specific orientation covering North and South units
* Charting system: Point Click Care (PCC)
* IV-C certification required for LPNs
* Required certifications: BLS Certification
* BSN not required
* Common diagnoses and patient types include CHF, Dementia, Ostomy, Catheter, Depression, Anxiety, Diabetes Mellitus, Hypothyroidism, Hyperlipidemia, High Blood Pressure, Hemiparesis secondary to CVA, Alzheimer's, Pain, Constipation, Muscle Weakness, A-fib, GERD, and Edema

The facility utilizes Point Click Care (PCC) as its electronic medical records system and maintains dedicated North and South units.
