Opportunity:

Physical Therapist - Home Health

Job Details:

* Provide in-home physical therapy services to patients in a home-based setting
* Perform physical therapy visits including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits
* Develop and implement individualized physical therapy plans of care based on patient needs and physician orders
* Deliver skilled interventions including gait training, balance training, therapeutic exercise, mobility training, and fall prevention
* Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
* Communicate patient progress, concerns, or changes in condition to the interdisciplinary care team
* Complete all required documentation accurately and timely in Homecare Homebase (HCHB)
* Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards
* Collaborate with clinical staff including RNs, PTAs, OTs, STs, and leadership on care coordination
* Provide clinical oversight of Physical Therapist Assistants (PTAs) when applicable and permitted under Colorado law
* Current, unrestricted Colorado Physical Therapist license required
* Valid driver's license and reliable transportation required
* Proficiency or willingness to learn Homecare Homebase (HCHB) required
* Home health experience preferred
* Familiarity with Medicare documentation and home health regulations preferred
* Experience working with geriatric and medically complex populations preferred
