Estimated Gross Weekly Pay: $2817.40

Opportunity:

Care Manager

Job Details:

* Every other weekend requirement - Saturday and Sunday, with a day off during the weekday
* Patient caseload of up to 25 patients per day
* No floating required
* Minimum 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Travel experience required
* First-time travelers accepted but must be screened
* Dress code: Business attire or scrubs
* Charting system experience required - Epic
* Must have experience with HMOs, IPAs, and similar organizations
* Must have experience assessing patients, Care Coordination, and Discharge Planning
* Computerized documentation experience required
* Required skills include: Acute Hospital, Admission Criteria, Care Coordination, CMS, CPT coding and billing, Department of Health, Medical Necessity Determination per Evidence-Based Guidelines, Discharge Planning, Disease Management, DRG, HIPAA guidelines, ICU, MS, Needs Assessment/Order DME, Plan of Care, and SDU/PCU/IMC/Obs
* Patients served include adults and geriatric populations
* Compact license must be in hand at time of submission
* RTO restricted to no more than 7 days
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day

The facility supports interdisciplinary care coordination and discharge planning, utilizing Epic as its primary charting system.
