Opportunity:

RN - Nurse Navigator

Job Details:

* Work 5 shifts of 8 hours per day on a day schedule
* No weekend requirements
* No floating requirements
* Minimum 2 years of experience required
* Experience with case management and navigation required
* Experience with Epic charting system required
* Primarily working with adult patient populations
* Primary patient diagnosis focus includes Sickle Cell disease
* Required skills include authorization forms, chart preparation, disability forms, follow-up phone calls, insurance verification, medication refills, pain assessment, patient health questionnaire/history/allergies, patient teaching, pharmacy calls/patient refill requests, referrals, and scheduling appointments
* Required certifications: BLS, CA RN license
* Pending license not accepted
* Blue scrubs required
