OT - SNF/Home Health

Job Details:

* Work across two care settings: Skilled Nursing Facility (SNF) and Home Health (HH) on an approximately 50/50 split
* Primarily located in the Beatrice, NE area with Auburn Home Health extending to that direction
* Day shift with variable scheduling
* Guaranteed 36 hours per week
* Supervise 1 Certified Occupational Therapy Assistant (COTA)
* New graduates are welcome to apply
* Utilize HCHB software for Home Health and NetHealth software for SNF documentation
* Productivity expectations: 80% productivity for SNF and points-based system for Home Health
* Therapy Supervisor available onsite
* Required certification: BLS (AHA or ARC)
* Post-acute care setting
