Opportunity:

LPN - Post Acute

Job Details:

* Work 12-hour night shifts from 6:00 PM to 6:00 AM, with some shifts from 10:00 PM to 6:00 AM
* Every other weekend and holiday requirement
* Unit operates with an average patient ratio of 1:22
* Units consist of 20 and 22 beds respectively
* Charting completed using Point Click Care (PCC) EMR system
* IV Certification required
* Location-specific orientation consists of 1 to 2 shifts
* Common diagnoses and patient types include dementia, diabetes, hypertension, chronic kidney disease, falls, and patients requiring ADL assistance
* Required certifications: BLS Certification
* BSN not required
