Opportunity:

RN - Case Management

Job Details:

* Day shift position Monday through Friday, 7:00 AM to 3:30 PM
* No weekend, holiday, or on-call requirements
* Manage a caseload of 8 to 10 patients per day
* Perform utilization review and discharge planning, including UR for NOAs
* Cover inpatient, observation, and swing bed areas
* Work with common patient populations including COPD, CHF, and debility for swing bed patients
* No clinical involvement required
* Utilize Meditech EMR system for charting
* Insurance portal experience required
* Minimum of 2 years of case management experience required
* CCM (Certified Case Manager) preferred but not required
* Required certification: BLS (AHA)
* BSN not required
* 2 to 3 shifts of unit/department-specific orientation provided
* Market Care Manager available on-site as a mentor and resource throughout the assignment
* Any color scrubs or business casual attire acceptable

This acute care Critical Access Hospital provides comprehensive inpatient services including observation and swing bed care. The facility supports case management staff with a dedicated market Care Manager available to serve as a mentor and answer questions during the assignment.
