Opportunity:

RN - Case Manager

Job Details:

* Work day shift Monday through Friday from 8:00 AM to 4:30 PM
* No weekends required
* Patient ratio of 1:14
* Work across two step-down units: CPCU (Cardiac Step-Down, 3rd floor) and PCU (Medical Step-Down, 4th floor)
* CPCU is a cardiac step-down unit serving CABG and TAVR patients, with many patients discharged home with cardiac rehab
* PCU is a medical step-down unit serving a variety of patients
* Perform case management assessments and discharge planning
* Work closely with consistent physicians and mid-level providers
* Minimum 1 year of current Case Manager RN experience required
* BLS certification required
* Active Kansas RN licensure required
* Meditech and Midas EMR experience strongly preferred
* Prior HCA hospital and HCA case management experience preferred
* Must be flexible and comfortable in a busy, fast-paced, constantly changing environment
