Opportunity:

RN/LVN - Home Health Intake Coordinator

Job Details:

* Day shift, Monday through Friday from 8:00 AM to 5:00 PM (5 x 8-hour shifts)
* Process home care referrals and coordinate amongst clinical staff, referral sources, and community liaisons
* Ensure patients are properly entered into the system, scheduled for visits, and all stakeholders are informed of patient status
* Receive and accurately document new referrals in a timely manner
* Interface with physician offices and community liaisons to assess and process referrals and admissions
* Verify patient insurance and explain benefit information to patients and case managers
* Answer incoming calls for intake patients and assist with overflow as necessary
* Distribute referrals to appropriate clinicians including intake data, essential background information, and plan of care
* Provide clinical guidance and identify educational opportunities for case managers
* Work independently, meet time deadlines, and manage multiple tasks simultaneously
* Participate in clinical work activities including faxing, scanning, emailing, and copying
* Minimum 1.5 years of experience required; candidates with clinic, home care, SNF, or hospital experience preferred
* Associate's degree in nursing or equivalent combination of education and related experience required
* Current RN or LVN license in the state of practice required
* Current BLS/CPR certification required
* Epic experience required
