* Provide hospice coverage when on-call
* Serve as Case Manager for assigned patients and actively manage caseloads and visit times in accordance with agency productivity standards

* Assess patient and caregiver needs utilizing the nursing process to provide quality care
* Develop and communicate individualized plans of care for each patient, ensuring patient and caregiver involvement
* Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective patient care
* Maintain high-tech clinical skills including IV use and maintenance, wound VAC, and complex wound care
* Collaborate with LPNs within their scope of practice to ensure plan of care is followed and patient needs are met
* Maintain up-to-date and accurate nursing documentation to support continuity of care across the healthcare team
* Follow agency protocol with McKesson processes including locking down visits, follow-up notes, and visit completion
* Perform patient and caregiver education on medication side effects, treatment options, discharge instructions, and reportable conditions
* Identify and document interdisciplinary patient needs including physical therapy and social work, and communicate to appropriate disciplines in a timely manner
* Accurately complete Agency, State, and Federal forms including HHABN and Medicare Notice of Non-Coverage

* Assess patient needs for HCA services and provide ongoing supervision in accordance with State and Federal regulations
* Transmit patient incident reports to QI Specialist in a timely manner as needed
* Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork is processed
* Coordinate patient care with physicians, providers, team members, and vendors to support the recommended plan of care
* Participate in monthly peer review audits and provide and accept constructive feedback professionally
* Proactively schedule, coordinate, and facilitate case conferences to maximize team communication
