Open Position: PT - Physical Therapist

Opportunity:

Physical Therapist - Acute and Extended Long-term Care

Job Details:

* Work 40 hours per week in rehabilitation services
* Provide physical therapy services in acute care, nursing home, and outpatient settings
* Float between acute/swing beds, nursing home, and outpatient services as needed
* Perform physical therapy evaluations and establish plans of care
* Participate in rounds and care conferences as indicated
* Communicate frequently with rehab team regarding patient progress, equipment needs, and discharge planning
* Work with typical rehabilitation equipment in various settings including patient rooms, hallways, outdoor areas, and rehab gym
* Use Cerner and NetHealth documentation systems
* Patient ratios of 1:1
* Prefer minimum 3 years of experience in medical setting
* Experience in hospital and/or nursing home setting required
* Required certifications: CPR and North Carolina Physical Therapy license
* Dress code: Business casual or scrubs of any color
* Schedule must be reviewed during interview with rehab manager

Facility:

This community hospital serves multiple counties and offers comprehensive healthcare services including 24/7 emergency care, acute inpatient care, rehabilitation, and long-term care. The facility features 24 acute/swing beds and 80 beds in their extended care center, which maintains a 5-star rating. The hospital is stroke certified and focuses heavily on rehabilitation services. The facility includes two outpatient clinics for continuum of care and features both indoor rehab gyms with plans for a new outdoor gym currently in development.
