Open Position: Social Worker - Licensed Social Worker

Estimated Gross Weekly Pay: $2657.00

Opportunity:

LCSW or RN Case Manager

Job Details:

* 5x8 shift schedule from 8:00 AM to 4:30 PM
* Will work every other weekend
* Block scheduling type
* Patient caseload includes med/surg, neurology, oncology, cardiac, and OBS patients
* Patient ratio of 20 patients per day
* Minimum 2 years of experience required within specialty
* Travel experience required
* Epic charting system experience required
* Acute hospital and home health experience required
* LCSW certification required
* Must be able to perform biopsychosocial assessments, crisis intervention, and discharge planning
* Experience with case management, care coordination, and interdisciplinary team collaboration required
* Must be comfortable working with adult patient population
* Skills in advance directives, medication assistance, and community resource referrals expected
* Free parking available on site

Facility:

This facility is a short-term acute care Level III trauma center with 252 staffed beds serving a daily census of approximately 250 patients. The facility is part of a well-established regional health system and utilizes Epic for charting. Interdisciplinary support includes physical therapy, social services, pharmacy, and transportation services.
