Opportunity:

Home Health RN - Full Time

Job Details:

* Monday through Friday schedule
* 36-hour weekly guarantee for the duration of the contract
* First week of assignment will consist of approximately 32 scheduled hours due to orientation requirements
* Provide skilled nursing care to patients in their homes
* Conduct patient assessments and develop and implement individualized care plans
* Coordinate with the interdisciplinary team to support quality patient outcomes
* Ensure timely and accurate documentation of all patient care activities
* Utilize clinical expertise and effective communication to manage home health visits throughout the service area
