Job Details:

* Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required
* Healthcare Provider Level BLS certification required
* Oregon LCSW (Licensed Clinical Social Worker) preferred
* Experience as a social worker in a hospital or medical setting preferred
* Previous hospice experience preferred and will be given priority consideration
* Position requires use of personal automobile for travel
* Must possess a valid driver's license and maintain auto liability/property damage insurance as required by law
* Must maintain personal vehicle in good working order
* Provide intermittent skilled nursing visit support and 24-hour on-call services
* Support patients and family members through the final stages of illness
* Services provided in home settings including patient residences, skilled nursing facilities, assisted living facilities, and adult foster homes
* Serve patients across Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties
