Opportunity:

LTC LPN - Per Diem

Job Details:

* Per diem shift schedule with shifts available Monday through Sunday
* May pick up shifts as desired
* No on-call or call back requirements
* Weekend and holiday availability required on a per diem basis
* Required to float to other units as needed
* Hours are guaranteed per week except during holidays when unit is closed
* Work across two units: 200 Wing (mixed occupancy including total assistance residents, short-term rehab, and end-of-life care) and 100 Wing (ambulatory and cognitively intact residents requiring less intensive care)
* 200 Wing staffing: 1 CNA to 8 residents on days; 3 CNAs plus nurse and recreational therapy on PMs
* 100 Wing staffing: 1 CNA to 12 residents; 2 CNAs and one nurse caring for up to 24 residents
* Must enjoy working with elderly residents and be able to work collaboratively as many residents require 2-person assist for transfers (Hoyer and EZ stand)
* Charting experience with Matrix preferred
* Minimum 2+ years of LPN experience in LTC/SNF setting required
* Active MN LPN License required
* AHA BLS certification required
* Associate Degree or Certificate required
* Professional scrub colors required
* Strong attendance and reliability are critical
