Pharmacist

Job Details:

* Variable shift schedule of 4 x 10-hour shifts, mostly nights from 8:30 PM to 6:30 AM
* Occasional day shifts from 10:15 AM to 8:45 PM
* Rotating days, generally 8 days on and 6 days off, including weekends
* Christmas Eve and Christmas Day coverage may be required
* No on-call responsibilities
* Minimum 1 year of inpatient pharmacy experience required
* EPIC experience required
* Graduate of an accredited school of pharmacy required; Doctor of Pharmacy (PharmD) degree preferred
* Pharmacist license in the state of practice required
* Required certifications: ACLS, BLS, and PALS from an approved vendor
* Verify, process, enter, and distribute medication orders in compliance with department and hospital policies, Joint Commission, and other regulatory guidelines
* Monitor patient medication therapies to ensure efficacy, safety, and cost-effectiveness
* Identify and troubleshoot potential medication safety hazards and document all interactions for medical review
* Conduct daily reports on medication utilization to identify areas for improvement
* Perform quality assurance procedures and participate in clinical rounds and unit-based medication management activities
* Collaborate with leadership to meet regulatory, accrediting, and licensing standards
* Supervise and direct support personnel

It is comprised of a 50-bed acute care medical center, a level IV trauma center, level II intensive care nursery services, an extensive physical rehabilitation program, an outpatient surgery and diagnostic center, and an advanced wound care center.
