LPN - Post Acute Care

Job Details:

* 12-hour night shifts
* Every other weekend required
* Required to work Christmas holiday
* Every third holiday rotation
* Patient ratio of 1:39
* Common diagnoses include Dementia, Parkinson's, and Major Depressive Disorder (MDD)
* Charting system: Point Click Care (PCC)
* BSN not required
* Valid LPN license required for submission
* Required certifications: BLS Certification
* 2-week cancellation notice policy required

This post-acute care facility operates a 39-bed unit providing specialized care for patients with complex neurological and psychiatric diagnoses. The facility utilizes Point Click Care (PCC) as its electronic medical records system and maintains a dedicated team focused on delivering quality long-term and rehabilitative care to its patient population.
