Opportunity:

RN - Home Health

Job Details:

* Work Monday through Friday from 8:00 AM to 5:00 PM with flexibility based on patient needs
* Scheduled for 1 weekend per month, with 1 day off prior to Saturday and 1 day off after Sunday
* Some on-call shifts may be scheduled on an emergency basis only
* Travel from patient to patient for home visits required
* Must have OASIS (Outcome and Assessment Information Set) experience
* Home Health and Hospice experience required
* Must be able to manage and adjust to changes in schedule
* Must have a valid driver's license in good standing
* BLS certification required
* EMR system used is HealthWyse (Careficient)
* Outpatient care setting
