Opportunity:

RN - Case Manager

Job Details:

* Minimum 2 years of acute care case management experience required with previous California experience
* Active California RN license required - no pending or temporary licenses accepted
* Required certification: BLS
* Epic charting system experience highly preferred
* No weekend shifts required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must remain flexible
* Patient ratio of 15 patients per day
* Serves patient age groups including adolescents, adults, and geriatrics
* Skills required include discharge planning, care coordination, utilization of InterQual and Milliman guidelines, ICD-10 coding, HIPAA guidelines, and CMS knowledge
* Flu vaccination required - no exemptions

This facility is a large acute care hospital with a daily census of 255 patients, offering a collaborative interdisciplinary environment with support from social services and transportation departments. The case management unit operates with a structured team focused on coordinating patient care, discharge planning, and ensuring compliance with regulatory and quality standards.
