Job Details:

* Work 8-hour day and PM shifts
* Every other weekend requirement
* Every other holiday requirement
* No on-call or call back available
* Patient ratio of 13:1 with 1 CNA and 1 Nurse
* Post-acute care setting with 75 beds
* Common diagnoses include CHF, heart failure, falls, hypertension, and diabetes
* Assist patients with all cares including transfers, toileting, and ADLs
* Charting system: Point Click Care (PCC)
* Required certifications: BLS Certification
* Active MN license required to apply, no temporary licenses accepted

The facility utilizes Point Click Care as its electronic medical record system and maintains structured staffing ratios to ensure quality patient care.
