RN - Case Management

Job Details:

* Seasoned travelers only - first-time travelers are not accepted
* Patient ratios: 1:25 on days and weekends
* Weekend rotation required (every other weekend)
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* No floating expectation
* California state license required; pending license accepted but must be active by start date
* Minimum 2 years of acute care case management and discharge planning experience required; behavioral health experience does not apply
* Travel experience required
* Charting system: Epic
* Dress code: Business attire or scrubs
* Patient age groups: Neonates, adolescents, adults, and geriatrics
* Interdisciplinary support includes physical therapy and social services
* Experience with assessing patients, care coordination, discharge planning, and utilization review required
* Ability to organize and participate in family meetings
* Participate in the development of treatment plans
* Experience with HMOs, IPAs, and similar organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, and protective services process required
* Must be able to independently arrange DME, SNF placement, and ambulance transport
* Computerized documentation required; Epic experience preferred
* Required skills include: acute hospital, admission criteria, benefits eligibility, care coordination, CMS, department of health regulations, discharge planning, disease management, DRG, emergency department, ICU, MS, needs assessment/DME ordering, plan of care, prior authorizations, surgical services, and Joint Commission/core measures/national safety goals
* Orientation hours: 40
* RTO restricted to no more than 7 days; all RTO must be requested upfront during submittal
* May be expected to float between sister facilities nearby; refusal to float may result in termination or forfeiture of guaranteed hours
