Case Management

Job Details:

* Minimum 2 years of case management experience required
* Acute care hospital case management experience required and must be current within the last year
* Travel experience required; first-time travelers accepted with clinical approval
* Every other weekend required
* No call required
* Float between units based on departmental need
* Patient ratio of 20 patients per day
* Charting system: Epic (required)
* Serves patient age groups including adolescents, adults, pediatrics, and geriatrics
* Interdisciplinary support includes social services
* Experience required with HMOs, IPAs, and similar managed care organizations
* Experience with advance directives, POLST, and protective services processes required
* Experience with discharge planning, care coordination, prior authorizations, appeals and denials, and benefits eligibility required
* Knowledge of CMS, DRG, HIPAA guidelines, and The Joint Commission/Core Measures/National Safety Goals required
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Compact license must be in hand at time of submission
* 2 references required covering 1 year within the last 3 years
* All RTO requests must be submitted upfront at time of submittal
* Free parking provided on first day; assignment parking provided during orientation
