Opportunity:

Home Health RN - Full Time

Job Details:

* Monday through Friday schedule
* Provide direct home health nursing care including managing a patient caseload
* Conduct home health admissions and complete OASIS documentation
* Perform wound care for assigned patients
* Oversee case management and coordinate high-quality care for home health patients
* Serve as primary caregiver for assigned patients
* Occasionally support preceptor in mentoring a nurse resident by assisting with patient care activities and clinical skills development
* Provide caseload coverage for preceptor while they focus on 1:1 training and onboarding for nurse resident
* Help foster a supportive, educational, and nurturing learning environment for nurse residents transitioning into home health nursing
* Deliver individualized, goal-oriented nursing care using the nursing process in accordance with physician orders
* Serve as Case Manager when acting as the lead discipline for assigned cases
