Job Details:

* 13-week assignment
* Rotating shifts with effort to provide a consistent pattern when possible
* Alternating weekends required
* No on-call required
* Block scheduling not required
* Patient ratio of 1:16
* Float between all three ministries required
* Conduct Emergency Department behavioral health assessments for patients of all ages including adolescents, adults, and geriatrics
* Perform care management discharge planning on medical/surgical floors
* At least 1 year of experience required
* Preferred experience in Emergency Department behavioral health assessments and care management discharge planning
* Community hospital experience preferred
* Trauma Level I and Level II experience preferred
* Proficiency with Epic charting system required
* Active LCSW certification required
* Compact license must be in hand at time of submission
* Skills required include case management, discharge planning, emergency department, medical surgical, psychiatric, and short-term acute care inpatient medical social work
* Scrubs: any color; OR/Critical Care/ED/Maternal Child scrubs provided
* No parking fee

This short-term acute care facility operates with 109 total staff beds and maintains a daily census of approximately 60 patients. The facility utilizes Epic as its primary charting system and supports a multidisciplinary care environment across several ministries.
