PT - Home Health / SNF

Job Details:

* Day shift with variable scheduling
* Guaranteed 40 hours per week
* Work week runs Sunday through Saturday
* Split setting position approximately 50/50 between SNF and Home Health
* Cover Beatrice SNF and Home Health in surrounding areas, also assisting with Auburn and Syracuse areas with PTO and census needs
* Supervise 2 PTAs
* New graduates are welcome to apply
* SNF software: Net Health; Home Health software: HCHB
* Productivity expectations: 80% for SNF and points system with 80 minutes per patient; approximately 27 points if full time in Home Health
* Therapy Supervisor available onsite
* No on-call or call back available
* Required certifications: BLS (AHA or ARC)
