Opportunity:

LVN - Home Health/Hospice

Job Details:

* Day shift position working 5 x 8-hour shifts per week
* Must be available to work weekends and 3 days during the week
* Provide care for assigned patients under the supervision of a Registered Nurse (RN) in accordance with established standards and nursing process
* Perform routine visits based on departmental specific policies and procedures
* Assist in the development of prioritized interdisciplinary plans of care for each patient
* Implement and coordinate plans of care utilizing appropriate organizational skills and reporting mechanisms
* Implement individual teaching plans including Patient Teaching Protocols and discharge plans
* Perform assigned clinical procedures including phlebotomy, injections, and drug screens
* Administer medications as directed
* On-call duties may be assigned based on location and agency operational needs
* Minimum 2 years of recent experience in the field required
* Current hospice experience required
* EPIC experience required
* Licensed Vocational Nurse (LVN) licensure in state of practice required
* Basic Life Support (BLS) Healthcare Provider certification required
* Arrhythmia certification preferred
* Valid Driver's License required and must be at least 21 years of age
