Job Details:

* Licensed Clinical Social Worker (LCSW) credential required - no exceptions
* Minimum of 2 years of experience required
* Patient ratio of 1:15 with day ratios of 1:7 and weekend ratios of 1:20
* Some weekends required as needed
* No on-call required
* Position may float between units
* Block schedules not required
* Orientation hours: 80
* Charting system: Epic experience required
* Scrub color: Business casual attire required for this role
* Serve patient age groups including adolescents, adults, and geriatrics
* Behavioral health patients may be located anywhere in the hospital, including the emergency department
* Perform biopsychosocial assessments and conduct initial evaluations
* Provide crisis intervention and individual counseling
* Facilitate discharge and contingency planning
* Participate in development and coordination of treatment plans
* Complete documentation according to plan of care including discharge summaries
* Work with multidisciplinary team to ensure continuity of care
* Assist patients with navigating community resources including social services, housing, and financial assistance programs
* Experience with 5150/involuntary holds, psychiatric disorders, and suicide/homicide/self-harm assessment required
* Compact license must be in hand at time of submission
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* May be expected to float between sister facilities nearby contracted facility

This short-term acute care facility operates with 108 total staff beds and maintains a daily census of approximately 100 patients. The facility utilizes Epic for charting and offers interdisciplinary support including physical therapy, respiratory services, interpretation services, phlebotomy, social services, rapid response teams, pharmacy, transportation, and unit secretarial support.
