Opportunity:

RN - Case Management

Job Details:

* Monday through Friday schedule from 8:00 AM to 4:30 PM
* Coordinate and facilitate care of patient population through effective collaboration and communication with the Interdisciplinary Care Transitions (ICT) team
* Follow patients throughout the continuum of care to ensure optimum utilization of resources, service delivery, and compliance with external review agencies
* Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs
* Enhance quality of patient management and satisfaction to promote continuity of care and cost effectiveness
* Integrate functions of case management, utilization review and management, and discharge planning
* Minimum 2 years of recent RN Case Management experience required, with experience in a LTAC or STACH setting
* Experience with MCG, discharge planning across acute care settings, and strong IDT collaboration required
* Texas RN or multi-state RN license required
* BLS certification through the American Heart Association required
