Open Position: RN - Case Management

Estimated Gross Weekly Pay: $2130.80

Opportunity:

RN - Case Manager

Job Details:

* Work day shift hours from 8:00 AM to 4:30 PM
* Monday through Friday schedule with weekend rotation (Friday through Monday)
* No night shift requirements
* Patient ratio of 1:20 for Case Management RNs
* Work primarily in case management units of approximately 20 beds each
* Utilize mobile computers and EMR systems including Meditech and CarePort
* Minimum 1 year of current Case Management RN experience required
* Perform discharge planning assessments and discharge planning
* Conduct goals of care conversations and apply palliative care knowledge
* Emergency department experience preferred
* Discharge planning experience preferred
* Must demonstrate strong communication skills and professional demeanor
* Must be adaptable to working across different units
