Home Health Physical Therapist

Job Details:

* Receive and review physician therapy orders for Home Health and Hospice patients
* Complete comprehensive patient evaluations and establish written treatment plans with short-term and long-term goals
* Provide skilled physical therapy interventions including therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
* Assess home safety and recommend equipment, assistive devices, or environmental modifications to promote independence and prevent falls
* Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes
* Educate patients, caregivers, and family members regarding treatment plans, home exercise programs, safety measures, and expected outcomes
* Maintain accurate and timely clinical documentation including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation
* Collaborate and communicate effectively with physicians, nurses, therapists, and other healthcare professionals involved in patient care
* Travel to patient homes and other care settings to provide therapy services
* Must hold current Physical Therapist licensure in the state of Montana
* Must possess a valid driver's license
* BLS certification required
