PT - Home Health

Job Details:

* Work day shifts from 8:30 AM to 5:00 PM
* Typically rotate Saturdays once per month
* Care setting is Home Health serving Yolo County, including Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto
* Minimum of 2 years of experience as a PT in an acute, home care, or rehab setting required
* Required certification: BLS
* Charting system: Homecare Homebase (HCHB)
* Dress code: Scrubs are not required; if scrubs are not worn, slacks (khakis acceptable) and a polo or button-up shirt are required; any color scrubs are acceptable if chosen
* Provide physical therapy services to patients according to a written physician's plan of care
* Assess, evaluate, and update therapeutic, rehabilitative, and functional status and participate in the development of the total plan of care
* Evaluate home environment and assess for muscle strength, mobility, gait, and range of motion with potential for rehabilitation
* Instruct patients, families, and caregivers in the use and care of therapeutic appliances
* Report patient reactions to treatment or changes in condition to the physician
* Initiate physical therapy programs and instruct personnel and family members in certain phases of physical therapy
* May train patients in the use of prosthetic devices
* Supervise homemaker and Home Health Aides participating in patient rehabilitation regimes
* Identify patient and family needs for other home health services and refer as necessary
* Prepare and submit clinical and progress summaries based on attainment of goals
* Provide physical therapy consultation to home families and caregivers when indicated
* Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other applicable regulations
* Mileage reimbursed at the GSA rate with prior approval
* Must have good oral and written communication skills and strong organizational skills
