Estimated Gross Weekly Pay: $2799.80

Opportunity:

Care Manager

Job Details:

* Variable 5-day workweek with department operations occurring 7 days per week
* Days off will vary and are not guaranteed to be back-to-back, though every effort will be made to schedule consecutive days off when possible
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Patient ratio of 1:25 with a daily census of up to 25 patients
* Patients include adults and geriatric populations
* Float to the Laguna Facility may be required
* Minimum 2 years of acute medical Care Manager experience performing Care Coordination required
* Travel experience required - no first-time travelers accepted
* California state license required; pending license accepted but must be active upon start date
* Epic charting system experience required - hard stop requirement
* Scrub color: Turquoise or Business Casual attire required
* Computerized documentation required
* Experience with assessing patients, care coordination, and discharge planning required
* Ability to organize and participate in family meetings
* Participate in the development of treatment plans
* Experience with HMOs, IPAs, and similar organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, and protective services process required
* Must be able to independently arrange DME, SNF placement, and ambulance transport
* Skills required include: Acute Hospital, Appeals and Denials, Care Coordination, CMS, Department of Health, Disability Case Management, Discharge Planning, Disease Management, DRG, Emergency Department, HIPAA guidelines, ICU, Needs Assessment/Order DME, OSHA, Plan of Care, Surgical Services, The Joint Commission/Core Measures/National Safety Goals, Worker's Compensation case management
* Trauma Level I and Level II experience preferred
* Case Managers do not perform Utilization Review
* RTO restricted to no more than 7 days
