RN - Case Management

Job Details:

* Day shift position with no night shift coverage available
* No weekend requirement
* Cover the entire hospital rather than one specific unit
* Patient ratio of 20-25 patients per RN
* Perform discharge planning and Interqual criteria to confirm patient status
* Float to any med-surge or ICU unit as needed
* Work with equipment including computers and phones
* EMR system: Meditech/Meditech Expanse
* Minimum 2 years of hospital case management experience required
* Active RN license required
* No required certifications beyond RN license
* Business casual attire required - no scrubs; plain white lab coat permitted if owned
* Best personality fit: flexible and comfortable asking questions

This facility is a full-service hospital located in the greater Houston area, offering a comprehensive Case Management and Social Work Department that covers all units throughout the main campus. The department operates with a dedicated team including case managers, social workers, case management assistants, a manager, and a director, all working collaboratively to support patient care and discharge planning across the entire hospital.
