Job Details:

* Weekend rotation schedule required
* No on-call responsibilities
* Coverage area includes Petoskey, East Jordan, Boyne City, Harbor Springs, and Mancelona areas with potential coverage extending from Mackinaw City to Mancelona along I-75 west
* Initiate and provide medically prescribed skilled nursing care and health counseling to patients in their homes
* Conduct initial patient visits to assess physical and emotional status, needs, and resources through observation, interview, and analysis of records
* Prepare nursing plans of care in accordance with physician treatment plans based on nursing assessment findings
* Provide patient teaching using Care Plan and Standards of Care
* 2+ years of hospice experience required
* Travel experience preferred
* Home Care Home Base proficiency required
* OASIS experience preferred
* Required certification: BLS (AHA)
* Michigan nursing license required
