RN - Case Management
Job Details:
* Seasoned travelers only - first-time travelers are not accepted
* Patient ratios: 1:25 on days and weekends
* Weekend rotation required - every other weekend
* California state license required; pending license accepted but must be licensed upon start date
* Requested time off limited to no more than 7 days
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Business attire or scrubs required
* Charting system: Epic
* Orientation: 40 hours
* Must have a minimum of 2 years of acute care case management and discharge planning experience - behavioral health experience does not apply
* Hospital experience required
* Experience assessing patients, coordinating care, discharge planning, and utilization review required
* Ability to organize and participate in family meetings
* Participate in the development of treatment plans
* Experience with HMOs, IPAs, and similar organizations required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, and protective services processes required
* Must be able to independently arrange DME, SNF placement, and ambulance transport
* Computerized documentation required - Epic system
* Patients served include neonates, adolescents, adults, and geriatrics
* Interdisciplinary support includes physical therapy and social services
* Required skills: acute hospital, admission criteria, benefits eligibility, care coordination, CMS knowledge, department of health, discharge planning, disease management, DRG knowledge, emergency department, ICU, med-surg, needs assessment/DME ordering, plan of care, prior authorizations, surgical services, and Joint Commission/core measures/national safety goals
* May be expected to float between sister facilities nearby the contracted facility
