Job Details:

* Acute Care Case Management/Discharge Planning position requiring 2 years of acute care experience
* Day shift patient ratio of 1:20 with a case load of up to 25 patients
* Every other weekend required with availability for holidays including commitment to 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* No floating required
* Business attire or scrubs accepted as dress code
* Epic charting system experience required
* Must be competent with InterQual guidelines and criteria for ED, Mother Baby, and NICU
* Experience with assessing patients, care coordination, and discharge planning required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis, advance directive, POLST, and protective services process required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, Medicare, and MediCal required
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Knowledge of Medicare guidelines required
* Experience with admission criteria reviews including ED, Mother Baby, and NICU
* On weekends, may be required to cover up to 2 units with focus on discharge planning
* Patient age groups include neonates, adolescents, infants, adults, pediatrics, and geriatrics
* Computerized documentation required
* Interdisciplinary support available including IV Teams, Physical Therapy, Respiratory Services, Pharmacy, Social Services, Rapid Response Teams, and more
* California state license required; pending license accepted but must be active by start date
* RTO restrictions of no more than 7 days per contract
* First-time travelers accepted but must be screened
* Travel experience required
