Opportunity:

Occupational Therapist - Home Care Full Time, Days

Job Details:

* Full-time, day shift position providing home care occupational therapy services
* Average patient visit exceeds 60 minutes, allowing meaningful and comprehensive therapy sessions
* Work with broad independence while collaborating as part of a cohesive interdisciplinary patient-care team
* Screen, evaluate, re-evaluate, and treat patients in a home health setting
* Assess patients' functional ability and provide direct treatments
* Select appropriate therapeutic modalities for individual patient needs
* Communicate with all members of the health care team regarding patient care
* Document evaluations, treatment goals, and objectives accurately
* Teach patients and family members the rationale for treatments and procedures
* Must hold a valid Occupational Therapist license in Vermont and New Hampshire or an unencumbered compact license
* Bachelor of Science in Occupational Therapy or postgraduate Occupational Therapy Certification program required
* Must have passed the National Registration Exam
* Experience in a home health setting preferred
