RN - Case Management

Job Details:

* 26-week contract assignment
* Day shift schedule with weekend rotation (every other weekend)
* Patient ratios: 1:25 on days, 1:25 on weekends
* No floating expected
* 40 orientation hours required
* Seasoned travelers only - first-time travelers will not be considered
* Minimum 2 years of acute care case management and discharge planning experience required
* Hospital experience required - behavioral health experience does not apply
* California state license required; pending license accepted but must be active by start date
* Travel experience required
* Charting system: Epic
* Dress code: Business attire or scrubs
* Computerized documentation required
* Must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* RTO limited to no more than 7 days; all RTO must be submitted upfront at time of submittal
* Required skills include: patient assessment, care coordination, discharge planning, utilization review, HMO/IPA experience, knowledge of DME, SNF, and ambulance transport arrangements
* Must be able to independently arrange DME, SNF placement, and ambulance transport
* Experience with family meetings, treatment plan development, and prior authorizations required
* Knowledge of CMS, DRG, Department of Health, and Joint Commission standards required
* Interdisciplinary support includes physical therapy and social services
* Patient population includes neonates, adolescents, adults, and geriatrics
* May be expected to float between nearby sister facilities; refusal to float may result in termination of assignment
