Opportunity:

REGISTERED NURSE (RN) - Home Health

Job Details:

* Monday through Friday day schedule with on-call rotation participation required
* In-person home health position with independent patient home visits
* Flexible day-to-day workflow structure with consistent weekday schedule
* Perform comprehensive patient assessments and develop individualized plans of care
* Provide skilled nursing care and appropriate interventions
* Monitor patient progress and report changes to the care team
* Educate patients and families on medications, treatments, and care plans
* Coordinate care with LPNs, therapists, caregivers, and providers
* Complete timely and accurate documentation in EMR (HCHB preferred)
* Active Registered Nurse (RN) license in Colorado or active NLC compact license required
* Valid driver's license, reliable transportation, and auto insurance required
* Strong assessment and critical thinking skills required
* Ability to work independently and manage a structured caseload required
* Comfort with documentation and EMR systems required
* Home health experience preferred but not required
