Opportunity:

Physical Therapy Assistant - Home Health

Job Details:

* Work 8-hour day shifts in a home health setting covering the greater Sacramento County area
* Guaranteed 40 hours per week
* See 5-6 patients per day with a productivity goal of 5-6 patients per day
* Treat a variety of diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
* Document using Cerner charting system
* Mileage reimbursed at the GSA rate with prior approval
* Required certifications: BLS (AHA)
* Must provide 2 supervisory references from the modality submitting to (charge nurse accepted) at time of submittal; 1 within 12 months and 1 within 3 years
* Copy of driver's license required at time of submittal to validate legal name
