Home Health PT - Seaside OR and Surrounding Areas

Job Details:

* 8-hour day shifts
* No weekend requirement
* No on-call requirement
* 1:1 patient ratio with up to 6 patients visited per day
* 30 productivity points standard
* Facility may call off up to one shift per pay period (every two weeks)
* Minimum 1 year of skilled home health experience required
* First-time travelers accepted
* Local candidates accepted within a 50-75 mile radius
* Traveler must have their own reliable transportation
* Occasional maximum travel radius of approximately 60 miles per day (60-90 minute total drive time)
* System groups clients to minimize drive and windshield time
* Business casual attire required; scrubs allowed but not required and no specific color
* Charting completed via EPIC Remote Client for Home Health (on laptop) and Rover phone app; OASIS documentation required
* Free parking provided
* Mileage reimbursement at IRS standard rate
* Required certification: BLS
* Skills required include acute care, outpatient clinic, rehabilitation, skilled nursing facility, body mechanics, cervicalgia, low back pain, post-op care, spinal conditions, joint replacements, neurological conditions, home exercise programs, and patient/family education
* Compact license must be in hand at time of submission
* 2 references required covering 1 year within the last 3 years
