Opportunity:

RN - Case Manager

Job Details:

* Work 8-hour shifts from 8:00 AM to 4:30 PM, five days per week (5x8s)
* 36 guaranteed hours per week
* Weekend requirement: Potential
* On-call requirement: Potential
* Float within Care Management department required
* Minimum 2 years of RN Case Management experience required
* Patient caseload ratio of 1:20 to 1:30
* Manage patients including Medicare, Medicaid, Commercial, and Unfunded populations
* EPIC EMR proficiency required
* Required certifications: BLS (AHA only)
* Virginia State License or Compact license required
* Scrub color: Eggplant Purple or Business Casual attire per dress code policy

This facility is a large 495-bed healthcare organization designated as a Level II Trauma Center. The case management unit maintains 20-30 beds with patient ratios averaging 1:20 to 1:30. The facility has earned an "A" grade in Patient Safety for nine consecutive years, reflecting its strong commitment to quality patient care and safety standards.
