Social Worker

Job Details:

* Serve as an Outpatient/Inpatient Social Worker responsible for direct supervision and supportive contact for high-risk patients
* Design and manage a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency
* Conduct assessments, develop service plans, acquire resources, monitor progress, and respond to emerging client needs
* Link clients with community social service providers, healthcare providers, substance abuse, and mental health providers
* Review patient cases in inpatient and outpatient services departments and review medical necessity of referrals to specialty providers
* Collaborate with an interdisciplinary team to assess care plans, monitor progress, and ensure safe and efficient hospitalizations
* Evaluate patients in the ED for medical necessity and care coordination needs
* Assess potential admissions and work with advanced practice clinicians and physicians to determine appropriate admission status
* Propose and implement outpatient plans for safe discharge from the ED with physician approval
* Coordinate care for patients with abuse, chronic pain, and frequent utilization issues
* Interact with patients of all ages, from newborn to geriatric, including individuals with mental health issues
* Work closely with providers, nurses, case managers, and physicians to ensure implementation of appropriate resources
* Master's degree in Social Work (MSW) required
* Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required
* Valid, current, full, and unrestricted professional license to practice LMSW or LCSW in any U.S. state, Puerto Rico, or U.S. territory required
* Must have and maintain current BLS certification by the American Heart Association
* Minimum of 3 years of experience in a clinical care setting required
* Experience and knowledge of discharge planning, case management, or utilization review processes required
* Demonstrated knowledge of electronic health record systems required
* Shared contractor housing only - no pets or family members allowed per facility policy
