Home Health SLP - Seaside OR and Surrounding Areas

Job Details:

* 4 x 8-hour day shifts
* No weekend requirements
* No on-call requirements
* 1:1 patient ratio with up to 6 patient visits per day; 30 productivity points standard
* Guaranteed hours: Facility may call off up to one shift per pay period (every two weeks)
* Minimum 1 year of skilled home health experience required
* First-time travelers accepted
* Patients served include adults and geriatrics
* Oregon state license required; compact license must be in hand at submission
* Required certification: BLS
* OASIS experience required
* Charting systems: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
* Attire: Business casual; no jeans; scrubs allowed but not required with no specific color required
* Provider is expected to have their own transportation
* Occasional maximum travel radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
* Mileage reimbursement provided at IRS standard rate
* Parking is free
* Locals accepted within a 50-75 mile radius
